Parent/ Carer Declaration for FSM children

*I understand that Fulbridge Academy will share my information with Peterborough City Council to check the named child/ren receive benefits-related Free School Meals (FSM) 1. This check will be based on the information supplied by me and I declare that all details I provided are true and correct. *I understand I can only book a place for my child/ren at one HAF provider and my child/ren can only attend at one provider during each holiday. *I understand that my email address will be shared with the HAF team so they can contact me directly to confirm my booking.

*The HAF programme is for children eligible under Free School Meals (FSM) economic criteria only (not Universal Infant Free School Meals, which is a universal offer for children from Reception to Year 2). *If you believe you are eligible and are not currently receiving Free School Meals for your child/ren, please contact the school office Please note that when you sign up for Easter holiday club for your FSM child, you will receive an email from EEQU to confirm your space.

Select the Christmas Holiday Club to book your place and make payment(if required).

Sign Up Deadline:

We run a Holiday Club during the Easter, Summer and Christmas holidays.

Information will be sent via the MCAS app explaining how to apply for a place.

Reasonable charges per day with discounts for the number of days booked.

No sibling discounts.
